What you need to bring to your assessment session:
- Photo ID
- Insurance card
- Is this a re-evaluation?
- Please bring previous assessment report and discharge summary
- Do you have court charges?
- Please bring police report, defendant case history, driving abstract, contact information of your probation officer, court, or lawyer.
- Any other court documents related to your case
Substance Abuse Disorder

ACTS provides integrated mental health services in an outpatient setting.
